This was a rare case of anastomotic pseudo-aneurysm of the distal anastomosis after the Bentall procedure, which is a fatal complication because of its risk of rupture and sudden death.
The presentation can be varied and often develop many years after surgery as in our case.
The patient was operated under cardiopulmonary bypass between the axillary artery and the femoral vein. A deep hypothermia and circulatory arrest was performed before sternotomy.
The intervention consisted on resection of the false aneurysm and interposition of a prosthesis to establish the continuity of the ascending aorta.
The postoperative course was favourable.
