Vignette Clinique
It is a female 2 months old infant admitted in pediatric department for
respiratory distress. She started to show dyspnea with failure to thrive
since the age of two weeks . Her oxygen saturation on room air is 95% in
both upper and lower limbs and pulmonary auscultation is normal. Cardiac
assessment shows regular tachycardia at 160 cycle per minut with a 2/6
systolic murmur over the the left sternal border and loud second sound on
pulmonary area. All peripheral pusles are well felt.
Echocardiography showed situs solitus with concordant atrioventricular
connection but the great vessel arising from the left ventricle seems
bifurcating laterally .
