Unusual NSTEMI


51 year-old man was admitted in Intensive Care Unit for anterior NSTEMI CV risk factors: HTA, dyslipidaemia.
EKG: Pathological Q waves in V1 and V2, négative T waves in anteriors leads Coronary Angiogram At 24 hours: There are LAD recent occlusion (blue arrow) with strange lesion in LM (orange arrow) (fig.1)

What is this strange lesion in LM? And what further exam (s) to confirm the diagnosis?
